While not required to file claims, members are strongly encouraged to enroll on the MetLife Portal to see their claims and enrollment status. For instructions on enrolling in the portal click on the button below. All members may call 1-800-GETMET8 and utilize the automated system to verify their benefits, request additional information or file claims.
Cook Associates: All dental, optical and supplemental claims are administered by DH Cook Associates. Claim forms can be found on the respective pages. Claim forms can be mailed in or e-mailed in at [email protected].
